A MESSAGE TO OUR PATIENTS
We are pleased that you have chosen Midwest Neurological P.C. to provide your care. We feel it is important that our patients are aware of the office policies as well as the financial policies of our practice and encourage you to discuss with us any questions you may have.
If your insurance company requires prior authorization for you to see a specialist, please contact your insurance company or referring doctor, whichever is applicable, before your appointment and bring your prior authorization number and prior authorization form if required.
We will file your insurance as a courtesy to you; but you are expected to pay your co-insurance/co-payments/deductible on the day of service. We accept cash, Visa/Mastercard or checks.
We do not file personal injury cases/auto accidents. Payment is expected at time services are rendered. You will be given a receipt to file your own claim or to give to your auto insurance company or attorney.
MEDICARE -- We are participating physicians with Medicare, and our fees are set with Medicare guidelines, however, Medicare will only pay 80% of the allowable charge and the additional 20% is due by the patient or their secondary carrier. Charges for services rendered to Medicare patients will be filed with the Indiana carrier.
Returned checks – If a check is returned to us for insufficient funds, an additional $20.00 handling fee will be applied to your account. If a check is returned after the second deposit, you will be required to pay by cash or money order.
Forms – We are often asked to fill out forms for our patients. This is done when the physician can work it into their day. There is a fee for filling out forms that is based on the amount of time it takes the physician to review your chart and complete the forms.
It is the patient’s responsibility to know what facilities and which physicians are part of their insurance company network. Prior to seeing another physician (specialist) for consultation, getting physical therapy, lab work, x-ray or surgery; -- YOU NEED TO VERIFY WITH YOUR INSURANCE COMPANY WHETHER A REFERRAL OR PRE-CERTIFICATION IS NECESSARY.
We will be happy to assist you in locating the appropriate facility or physician as well as to do the authorization/pre-certification for you if possible; -- HOWEVER, ULTIMATELY IT IS YOUR RESPONSIBILITY TO SEE THAT THIS HAS BEEN DONE PRIOR TO TESTING OR CONSULTATION.
We apologize for the inconvenience, but it is IMPOSSIBLE for us to know the guidelines for the many different insurance plans! If we work together, we can assure better continuity of care and insurance coverage utilizing your highest level of benefits available.
Midwest Neurological P.C. is committed to providing the best quality of care to our patients. In order to accomplish this, our staff tracks all orders for testing, therapy, or referrals made to another physician/facility by one of our physicians. Due to the fact that this is a very time consuming process, if our staff has to contact you regularly because you have failed to follow through with our recommendations, you will risk the possibility of being terminated from our care. Therefore, if you are unable or decide not to follow through with the physician’s recommendations, please notify our office at (812) 476-7523.
Our telephone hours are Mon-Thurs 9 a.m. - Noon and 1:30 p.m. - 4 p.m. Fridays 9 a.m. - Noon and
1:30 p.m. - 3 p.m. We have an on call physician available at all times. However, we ask that they only be paged for emergencies. No controlled substance medications (pain meds) will be refilled after office hours.
Out of respect for other patients who need appointments, call our office at least 24 hours prior to your appointment time if you need to cancel or reschedule. We charge a $50.00 fee to new patients who fail to show for an appointment or fail to give a 24-hour cancellation notice and we charge a $25.00 fee for patients with follow-up appointments who fail to show or give a 24-hour cancellation notice. Our office reserves the right to terminate the patient/physician relationship at any time for non-compliance and/or intolerable behavior. Non-compliance includes failure to keep appointments without notifying the office of cancellation/rescheduling within 24 hours of scheduled appointment time, or failure to follow through with physician recommendations. Intolerable behavior includes, but is not limited to, shouting, profanity, verbal or physical abuse or threats.
